How to prepare for your appointment.

Please complete the New Patient Registration Form.

Download new patient form

This contains important personal and clinical information for your treating Specialist.

Ensure that you have an up-to-date referral letter from your GP so that you may claim a Medicare Rebate.

Please bring your MRI films/reports, blood test results and relevant past medical records to your consultation. If you are a migraine or headache patient, please bring along (or email) a copy of your Headache Diary.

If you require a Specialist Medical Report to be completed, please inform our administrative staff so that an appointment slot can be allocated for this.

A note about fees.

Private Fees are payable in full on the day of service.

We do not Bulk Bill.

We do not see patients for the purposes of worker’s compensation or TAC related claims.
